Hypertensive Urgency
  • (ACEI) (12.5 - 25 mg) onset 15 min ระวังในผู้ป่วยไต
  • Amlodipine (CCB) (5 - 10 mg) onset 4-6 hours
  • Prazosin (AlphaB) (1 - 2 mg) onset 1-2 hours ระวัง 1st dose effect
  • Hydralazine (10 - 20 mg) IV q 15 mins (max 40 mg)

  • 1 hr , ลด SBP ลง < 25%
  • 2 - 6 hr , to 160/ 100 mmHg
  • 24 - 48 hr , to baseline
Acute aortic dissection SBP 100-120 mmHg , HR 50-60
Hemorrhagic stroke 160/90 mmHg , MAP 110
Ischemic stroke (ให้ rtPA) < 185/110 mmHg
Ischemic stroke (ไม่ให้ rtPA) < 220/120 mmHg
Severe preeclampsia, eclampsia < 160/100 mmHg

CKD without albuminuria + microalbumin < 30 mg/day < 140/90 ( ++ II )
CKD with albuminuria + microalbumin ≥ 30 mg/day < 130/80 ( + III )
เคยมี Lacunar stroke < 130/80 ( + II )
เคยมี Coronary artery disease 140 - 115 / 90 - 60 ( + II )

Block voltage-dependent calcium channel -> relax vessel smooth muscle
Amlodipine2nd gen Dihydropyridine
Dihydropyridine จะกระตุ้นเฉพาะ arterial vascular smooth muscle

improve insulin sensitivity + type 2 diabetes mellitus and/or renal impairment + less ankle oedema
