Mild head injury | GCS 13 - 15
  1. Low risk (15) และต้องมีครบทุกข้อ --> D/C!!!
    1. GCS score of 15
    2. No headache
    3. Scalp injury : bruise or laceration
  2. Moderate risk (13-14)
    • GCS 13-14
    • GCS 15 + อย่างน้อย 1 อย่าง
      1. Vomiting < 2 ครั้ง
      2. Hx Loss of consciousness
      3. Headache
      4. Post-traumatic amnesia
      5. Transient loss of consciousness (seconds)
      6. Risks of coagulopathy
      7. Drug / alcohol intoxication
    • Tx: (วิธีที่ 1) ให้ CT + Admit observe 6 hr --> D/C!!!
    • Tx: (วิธีที่ 2) ให้ Admit observe + observe neuro sign q 1 hr จนครบ 24 hr + F/U 1 wk ดูอาการ
  3. High risk
    1. Score 13-14 หลัง observe 1-2 hr
    2. สงสัย Open skull or base skull fx
    3. Vomiting > 2
    4. Decrease score  ≥ 2
    5. Focal neuro deficit
    6. Post-traumatic seizure
    7. Age  ≥ 60 yr
    8. Tx: (วิธีที่ 1) ให้ CT + Admit observe 6 hr --> D/C!!!

Moderate head injury | GCS 9 - 12

Severe head injury | GCS 3 - 8
  1. Endotracheal intubation if GSC  ≤ 8 or จำเป็นต้อง protect airway
    2. ตรวจร่างกายมี Signs of transtentorial herniation
    • unilateral dilated fixed pupil
    • abnormal respiration
    • decerebrated or decorticated posture
  2. Hyperventilation ; RR 16-20 /min --> Keep PaCO2 30-35 mmHg
    ข้อบ่งชี้
    • signs of transtentorial herniation ได้แก่
    • Rapid deterioration

  3. NSS , LRS , Acetate
    • maintenance rate

  4. Mannitol 1 gm/kg drip in 15 mim
    เช่น 20%Mannitol 250 ml IV drip in 15 min
    ถ้าไม่มี mannitol อาจจะให้ furosemide 0.5-1 mg/kg IV แทนได้

  5. ยากันชัก for Early post-traumatic seizure ให้เฉพาะในกลุ่ม "เสี่ยงสูง" 
     Phenytoin 15-20 mg/kg drip in 30 min (ไม่เกิน 50 mg / min) then 5 mg/kg/day นานจนครบ 7 วัน
    Dilantin 1000 mg IV drip in 30 min then
    • 100 mg IV q 8 hr จนครบ 7 วัน or
    • Dilantin (100) 1*3 po pc จนครบ 7 วัน
    1. Immediate post-traumatic Seizure or U/D Epilepsy
    2. Post-traumatic amnesia นานกว่า 30 นาที
    3. ไม่ชัก แต่มีสิ่งที่เข้าได้ต่อไปนี้
      1. Acute SDH , EDH , ICH or Cerebral contusion (ในกรณีที่ CT scan)
      2. GCS < 10
      3. Penetrating head injury
      4. Depressed skull fracture > 1 cortex
      5. Chronic alcoholism
  6. Closed head injury ไม่จำเป็นต้องให้ ABO ถึงแม้จะมี fracture base of skull
  7. Tetanus toxoid: 0.5 mL IM (boost)
